All Registrations will be held at Hueytown Middle School. Please contact Student Services at 205-379-2165 with questions. Registration is closed each day from 11:30 am – 1 pm for lunch.

• Locker & Supply Move in Day: Aug 11, 4 pm – 7 pm

-  Proof of Residence Required: e.g. Mortgage Statement or Lease
-  Two current utility bills (water, electric or gas ONLY)
-  Proof of Legal custody, in case of divorce or other custody issue
-  Parent's Valid Driver's License or state issued ID card
-  Up-to-date Valid Immunization (Blue Card)
-  New Students: Birth Certificate
-  Social Security Card
